Initial Comprehensive RN Assessment & Care Plan

The first step in beginning care management services with Elite Visiting Nurse. This assessment helps identify the client's medical needs, safety concerns, current support system, and recommended level of ongoing care.

Includes: 

  • 2-3 hour in-home assessment

  • Medical history review

  • Medication reconciliation

  • Functional and cognitive assessment

MONTHLY CONCIERGE RN CARE MANAGEMENT PACKAGES

The Essentials Package

Ideal for independent seniors who need occasional professional oversight.

Includes:

  • Ongoing care plan review and updates

  • One in-home RN visit, up to 90 minutes

  • Medication reconciliation

  • Wellness assessment

  • Coordination with physicians

  • Up to 2 hours of care coordination

  • Two family update calls

  • Priority phone/text access during business hours

Best For:

  • Seniors living independently

  • Families wanting peace of mind

  • Long-distance adult children

The Advocate Package

Our most popular package for clients and families who need consistent RN oversight and healthcare coordination.

Includes:

  • Two in-home RN visits each month

  • Medication management

  • Physician appointment preparation

  • Attendance at one physician appointment, if needed

  • Care coordination

  • Specialist referrals

  • Home safety review

  • Four family updates

  • Unlimited email communication

  • Priority scheduling

  • Recent hospitalization

Premier Concierge Package

White-glove nursing advocacy for clients and families who need a higher level of support

Includes:

  • Weekly RN visits

  • Unlimited care coordination

  • Accompaniment to physician appointments

  • Hospital advocacy

  • Emergency room support

  • Rehabilitation coordination

  • Family meetings

  • Monthly written care reports

  • 24-hour priority response*

  • Coordination with attorneys, financial planners, and home care agencies

*24-hour priority response is subject to availability and the terms outlined in the Elite Visiting Nurse service agreement.

Elite Visiting Nurse does not provide emergency medical services. Clients should call 911 for medical emergencies.

Designed for clients returning home after hospitalization or rehabilitation.
